Advance Decision Form

An Advance Decision Form—also known as a living will or advance directive—is a legal document that allows an individual to outline their preferences for medical treatment in case they become unable to communicate or make decisions in the future. This may include decisions about life-sustaining treatment, resuscitation, and other healthcare choices.
